Description:
Quick action local anesthesic and anti-arrhythmic agent.

General Description:
LIDOCAINE 2% is an anesthesic which belongs to the amides group, is indicated to local and external use and to treat ventricular arrhythmias; specially paroxistic ventricular tachycardia.
LIDOCAINE 2% is considered a quick action local anesthesic. It is more powerful and it has more insensitivity time than Procaine.
Once applied, its estimated latency time is between 3 and 5 minutes, keeping the infiltrated zone or structure free from superficial sensitivity during 1 to 2 hours.
The addition of epilephrine to lidocaine prolongs the action and decreases its toxicity due to the slowness of the absorption.
LIDOCAINE 2% is a watery injectable solution, which contains the equivalent to 20 mg of Hydrochlorate Lidocaine every each ml of sterile injectable solution.

Bioactivity:
Lidocaine blocks the excitatory conduction of the spinal nerves through neuronal membrane stabilization. This remains in depolarization stage “O” avoiding the nervous impulse transmission.
LIDOCAINE 2% is considered as an IB type anti-dysrhythmia agent (membranes stabilizer).
At therapeutic doses, LIDOCAINE 2% provokes attenuation in the stage 4 of diastolic depolarization; it increases its automaticity and diminishes or does not vary excitability in the membrane. This effects occur at serum levels, therefore, automaticity in the sino-auricular node will not be inhibited and it will have a little effect over the auriculoventricular node or Hiss-Purkinje node conduction.


Indications:
LIDOCAINE 2% is indicated in the following cases:
• Any kind infiltrative anesthesia.
• Epidural anesthesia and/or analgesia.
• Subarachnoid Blocking.
• Para-vertebral anesthesia.
• Intercostal nerves blocking.
• Intrapleural anesthesia and/or analgesia.
• Intravenous regional anesthesia (Bier Blocking).
• Brachial plexus anesthesia
• Mandibular nerve infiltration, after mandibulectomy.
• Continuous epidural anesthesia and/or analgesia.
• Intra-articular infiltration.
• Peripherical nervous blocking (perineural or troncular) in equines, bovines, obstetrical, urogenital and corrective surgery.
• Diagnostic troncular anesthesia and ocular surgery.
• Heart dysrhythmia control, specially paroxistic ventricular tachycardia, ventricular arrhythmias and premature ventricular complex.

ADMINISTRATION:
Tissue infiltration, IM, SQ, IV, Intra-articular and Epidural injectable. Specific doses depend on the usage and application conditions of the product.

Metering:
Lidocaine 2% must be applied with needle and hypodermic syringe. Apply Lidocainde 2% as parental injection, through intramuscular, perineural, SC and/or intradermic infiltration in tissues. The intravenous method will be used to treat arrhythmias or for regional anesthetics techniques (Blair blocking).

Local or Regional Anesthesia: 3 mg/kg, equivalent to 0.15 ml/kg.

Epidural Anesthesia produces regional bloking; it affects motor, sensitive and sympathetic fibres. This procedure is used for epigastric region surgeries, orthopedic hip surgery, perineum, back members, pelvis, soft tissue surgery in back members, rachidian analgesic, caesarian surgery and obstretic movements. In this case, Lidocaine 2% original concentration is suggested. Apply no faster than 0.5 ml every 20 seconds.

• Dogs and Cats: 1 ml/4.5 kg blocks up to L1; 1 ml/3.4 kg blocks up to T5.

• Pigs: 1ml every 40 cm spinal cord, then add 1.5 ml every additional 10 cm spinal cord.

Sheep and Calf: from 8 to 12 ml according to the animal size and to the region to be made insensitive.

• Cows: back anesthesia – 4 ml.

Front anesthesia – If it is necessary the animal decubitus through this via, use 60 ml Lidocaine 2%.

Here, the animal should stay still for 3 or 4 hours to avoid autotrauma.

• Horses: apply 5 to 7 ml Lidocaine 2% (1 ml every 100 kg aprox.).
In case of higher doses, wait for animal’s decubitus.
These doses must be reduced in case of old animals or where an anatomical reduction of the light in the medullary channel is perceived.


Arrhythmias and Disrrhythmias Treatment

• Dogs: start with a 2 to 4 mg/kg bolus via slow EV, then a 25-80 ug/kg/min goteo.

• Cats: apply with care because cats are very sensitive to Lidocaine. Start with an initial 0.25-0.75 mg/kg bolus via slow EV, and then continue with a 10-40 ug/kg/min dripping.

• Horses: give an inicial 1-1.5 mg/kg bolus via slow EV, and keep constant intravenous infusion according to needs.

Bier Blocking: Once the tourniquet has been made, apply 10-20 ml Lidocaine 2% in any superficial vein.

Administration:

Contraindications :
Lidocaine could be used in infected tissues (abscesses) or in tissues with a trauma with inflammation (haematoma), due to the fact that under particular circumstances the tissues have a pH slightly acid, which hinders the hydrolysis in the acid salt of the anesthesic and the resulting release of alkaloid.
Lidocaine is contraindicated in: patients with know hypersensitivity to amides family local anesthesic; patients with sinoauricular, auriculoventricular or severe interventricular blockings.

Precautions:
In case of exceeding the indicated doses in more than 4 times the one described for the intravenous via.
If the indicated doses are exceeded in more than times the described for the endovenous via in arrhythmias maintaining, the presence of signs such as depression, ataxia, muscular tremors, nausea and transitory vomits. If an endovenous bolus is quickly given, it may occur a hypotension period is not often described.
Before injecting Lidocaine 2% in the tissues, the aspiration maneuver should be done, corroborating not to inject the drug in a blood vessel, which would impede to reach the desired effect.
Felines are especially sensitive to Lidocaine anti-arrhythmic effects; therefore, it should be used with precaution in those patients.
Lidocaine should be used with precaution in those patients with hepatic disease, congestive cardiac failure, shock, hypovolemia, severe respiratory depression, or hypoxia.
In patients susceptible to suffer from malignant hyperthermia, lidocaine should be administered under intense monitoring.

Warnings:
• In case of peridural or subarachnoid administration, do not exceed an administration speed of 0,5 ml every 20 seconds.
• Verify that the inviolability system of the product and its storage conditions before usage are the adequate for the product.
• Do not mix in the same flask or syringe with any other compound different from the suggested.
• In all the cases, the doses and frequencies are to be determined by the veterinarian in charge, who will determine the quantities according the requirements of the structure to be anaesthesiated and the kind of maneuver to be done.
• Under no circumstances should be product be open or transferred neither the content of the product should be exposed to environment, handled with maneuvers that put in danger the proper conservation of the product and the inviolability of the flask.
• Keep the product between 4 and 25ºC, away from direct sunlight, in a dry and hygienic place.
• Recyclable container. Destroy after use and throw in residues collectors.
• Protect the environment.
• Always ask your veterinarian.
• Do not eat. Keep away from the reach of children.
